When requesting hot water from the tap, the radiators also heat up, or vice versa: when requesting heating, hot water comes out of the tap.
Cause: The valve actuator does not reach the correct position and leaves both paths partially open at the same time.
Check if there is a clear movement or sound in the gas valve when switching modes; if there is no movement, point to the motor.

The domestic hot water takes time to come out or loses pressure when a tap is opened in front of the heating.
Cause: The valve must switch immediately to DHW upon detecting demand, and if it gets stuck halfway, it distributes the flow between the two circuits.
Close the radiators manually for a few minutes and check if the hot water output improves; if it does, the valve is not fully switching.

You can notice or hear the boiler trying to move something inside every time it changes mode, but it doesn't actually do anything.
Cause: The actuator motor is spinning freely or has lost power to move the valve body.
With the boiler turned off and cold, check if the actuator shaft turns by hand normally or if it is stuck.

There is a ring or water droplets in the area where the three-way valve is installed, without leaking from any other place.
Cause: The seals of the valve body lose tightness with use and circuit pressure.
Dry the area and check after one heating cycle and one domestic hot water cycle to see in which mode the humidity appears.
