Listen to see if the actuator hums without moving when you open a tap: if it makes noise and doesn't turn, the problem is mechanical and confirms the need for replacement.

There is a water leak right at the body of the valve, not at the pipe connections.
Cause: The internal seals of the body lose their tightness with years of use and the pressure of the installation.
Tighten the nuts on the connections before assuming it's the valve: a leak in the thread can easily be mistaken for a leak from the body.

The actuator moves but makes a strange noise, like a clicking or metallic rubbing sound.
Cause: The motor gear is worn and does not mesh cleanly with the valve shaft.
Remove the actuator cover with the boiler unplugged and check for any plastic debris from the gear: if there is any, it's not worth repairing, it needs to be replaced.

Cause: The actuator loses synchronization with the microswitches that inform the PCB of its position.
Check the display or the LEDs on the PCB to see if it shows a three-way valve error before disassembling anything: many models indicate this.
